Yuzuriha is a slim young woman of average height, with dark hair (depicted as purple in the anime) usually tied up in a bun and secured with a hairpin. After her battle with Ran, it is revealed that her body is covered in numerous scars from her past life as a Kai kunoichi. She concealed these scars by coating her body with a layer of mucus that mimicked the appearance of smooth skin.

At first, Yuzuriha wears the traditional kunoichi outfit, consisting of a sleeveless black garment (purple in the anime), a white obi wrapped around her waist, and a revealing hakama that exposes her thighs. Her top is often worn loosely open, showing much of her chest. She carries kunai strapped to both arms, a bandage on her right wrist, and a bracer on her left. Around her waist, she keeps bamboo containers filled with potions used for her ninjutsu, along with a wakizashi whose scabbard is wrapped with two ropes.
After arriving in Hōrai and experimenting with different outfits, she changes into a green cheongsam decorated with yellow peony patterns, featuring an opening at the chest, and pairs it with black kung-fu style flat shoes. When her outfit is later damaged by Ran, she replaces it with another cheongsam.
A year after escaping the island, Yuzuriha adopts a new appearance, letting her hair fall into a ponytail instead of wearing it pinned up. She is also seen wearing a floral-patterned yukata layered with a dark haori.
